deductibles, or services not covered by your health care plan.
Payment may be made by cash or check.
Many health care plans require the
patient to obtain a referral from their primary care doctor for
every visit to the specialist, including follow up visits, and we
cannot bill your insurance company for our services if you do not
provide us with this referral. Your doctor may mail or fax the
referral directly to us, or you can bring it with you.
